The application must be completed by the manufacturing company for Feed into Malaysia (Pet Food / Animal Feed / Feed Additive Containing Animal-Based Raw Materials) which is to be exported to Malaysia for the FIRST TIME. 2. A separate form must be used for different manufacturer. 3. All the questions in the application form must be completed or mark as not applicable.
